Instructions to submit a form for a discretionary award

1. Log in or create a new account at https://staging.lilynyc.org (bottom right)
2. Click APPLY
3. Enter access code
4. Enter award amount
5. Enter program or general operating support
6. Enter nominating board member. If it’s a nomination by all board members, please use foundation
7. Write a few sentences about the nature of the organization
8. Attach a copy of 501c3 form if organization isn’t registered in GuideStar
